Quick Guide to Aciclovir for VZV

Aciclovir is an important treatment for varicella zoster virus (VZV). It may be used to treat herpes zoster (shingles) but it can also be used to treat varicella (chickenpox) in adults, or severe infections in children.

Herpes zoster 

Chickenpox 

Oral Aciclovir

• Start within 72 hours of rash onset.
• In zoster infections, it can reduce the severity and duration of pain and may prevent post-herpetic neuralgia.
• Standard dosing (12+ year olds): 800 mg five times daily for 7 days. Adjust in renal impairment.

IV Aciclovir – reserved for severe cases:

• Immunocompromised patients with chickenpox, disseminated herpes zoster or severe herpes simplex
• Trigeminal nerve involvement, extensive eczema herpeticum, herpes infection of the brain (encephalitis) or herpes infections in newborn babies.
